健保卡遺失補辦 Renewing NHI IC card

l 申請補辦健保IC卡時，請先填寫「請領健保IC卡申請表」，申請表可至網頁下方下載：
Please fill in NHI IC card application form before applying to renew NHI IC card. You can download the application form below.

l辦理地點：
1.健保局各分局或各地聯絡辦公室
2.全省各郵局櫃檯

l辦理所需證件與費用：
1. 工本費新台幣200元
2. 最近3個月內2吋半身、脫帽不戴有色鏡片之照片乙張
3. 國民身分證、居留證或其他身分證明文件影印本 (貼於申請表背面)
Required Documents：
1. NT$200
2. 2" front, hatless, white-background photo
3. 2-sides：Photocopy of ARC

l申請核發時間：
健保各分局現場發件：30-60分鐘內完成。
郵寄申請 (掛號信)：7個工作天內可收到。
After application you have to wait：
1. 30 to 60 minutes if you went apply at Bureau of NHI branchs
2. in 7 days if you apply by sending registered letter.

l身分驗證：
如親自到健保局各分局現場申請補換健保IC卡時，應攜帶可證明身分文件正本(居留證) 等，如果委託他人辦理，則申請人與代理人之可證明身分文件皆應備齊。
Identity provement：
You should carry an official document which could prove your identity (such as ARC) with you while applying renewing NHI IC card on your own at Bureau if NHI branchs. If you authorize an agent to apply for you, the agent should show both of yours and his/her official documents which can prove your identities.
